Katherine Bain graduated from Washington University School of Medicine in 1925, the only woman in her graduating class. After completing her residency at St. Louis Children's Hospital in 1927, she joined her brother-in-law Park J. White in private practice. They ran the first racially integrated pediatric medical facility in St. Louis and fought to have black physicians admitted to the American Academy of Pediatrics. In 1940, Katherine Bain left St. Louis to work for the U.S. Department of Labor. She spend the rest of her career in government service, mostly with the Department of Health, Education and Welfare.
